Plan usage in the title bar

- Plan usage at a glance. The title bar shows the 5-hour or weekly limit closest to running out for the agent you're working with. Click it for every window and its reset time.
- Context in every pane. Claude Code and Codex panes show how full their context window is, next to the pane title.
- Complete Usage history. Tokens spent under connected accounts now count, not just your default sign-in.
